A contract is not considered complete unless it is returned, in it's entirety, with all necessary supporting documentation and any required booking fees. If the contract is accepted you will receive a fully executed copy in return. Please contact us with any questions.

Embryo Transfer - To secure a recipient for the 2023 season, please complete the embryo transfer contract and return ALL documents to our office with the $1,500 booking fee for each pregnancy desired. Recipients will not be reserved if all documents are not received in our office.
